Root Cause Analysis and its use in State Developmental Disabilities Agencies

This introduction to root cause analysis and its adaptation to developmental disabilities systems was commissioned by HSRI in 2002 in an effort to assist state developmental disabilities program administrators enhance their risk management and quality improvement activities. It is designed to provide guidance on the use of root cause analysis to those who manage programs and larger systems that serve and support people with developmental disabilities.
